Two flat tires in two weeks... What am I doing wrong?

I'd say I ride a mix of 60/40 road to gravel. First flat looks like it was tiny glass shards through the sidewall, second flat was the tiniest piece of wire (almost like guitar string like) through the tread. Tires are WTB Nano 700x40C and psi is at ~40. Is this just crap luck or am I doing something wrong? Should my psi be lower? Are there puncture resistant gravel tires? I'm currently looking into thorn resistant tubes.
