Arms should hang straight down from your shoulders with just about every club sans your woods. Even then you should only be extending them slightly. Armpits over knees over balls of your feet. Another que i would give you is to feel like the club head never gets close to your right shoulder at the top. Meaning, your wrist structure at the top is lacking and without it you need to fling the club back in front of you on the DS and that causes your OTT move.

Yw. Also better setup, closer to the ball, will help your follow through. The reason you lose your balance at the end is because your mind knows you need to get closer to hit the ball, so it moves your body at the last second to do that.
