Lens prescription

Hello everyone!
Im trying to buy some glasses online.
Im not really sure if i know my lens prescription or if i do have it. I have a receipt from the last glasses I bought and it says
SV sphcyl pl - 4.00/0.12-2.00
Is this what i need to know to buy glasses online or is this totally a different thing?
The glasses im trying to buy has the prescription tab where it has the
OD (SPH) and (CLY).
any feedback is appreciated.
