Do PS4 trophies carry over to PS5?

I have completed the main quests on PS4 base game as well as more than half of the Tales and collectibles. I want to get the Platinum trophy now.
Currently planning to buy a PS5. My question is do the save files and trophies carry over from PS4 to PS5 if I buy the PS5 Director's Cut? Or would I have to play the game from the very beginning? Thanks!
