What causes blisters? Friction? Vacuum?

So I see people using vacuum hangers and some use tape, some use the water trick, and I’ve seen bd use just lotion. When the water trick is discussed, people say to eliminate any and all air bubbles. But air is present with tape and lotion, no? And I don’t see many people getting blisters from pumping. So, what is it that causes blisters when using vacuum cups?
