Osmosis Jones meets open world land box RPG

Think of a game like skyrim, or red dead redemption two. The open world, free to play with under arching story in the background
But instead of of the old west, or some fantasy world with dragons, you're a white blood cell protecting the "world" (a body) from the invasive threats.
Context clues surrounding your host would be everywhere darksouls style, but the core mechanic would have you exploring dungeons (arteries) and helping npcs (gut bacteria, red blood cells, etc) to eventually save the world
I'm sure this is an old hat idea, I just wanted to get it down
