Capital Blue insurance for Top surgery???

I'm trying to find any information I can on this particular insurance and gender affirming surgery. So far, I'm finding next to nothing.

Does anyone know anything about this particular insurance and covering top surgery???

Or what about just generally, is it typical for health insurance to cover any parts of top surgery??

I have a rather large chest, and I'm just done with them. I want them gone. But I'm also a parent to 3 kids and have little money and not in a place we'll be able to come up with lots of money anytime soon.
