Are there any guys that have been on T for 5-10+ years that have NOT gotten a hysto?

I’ve been on T for almost 3 years now, and I am 18. I feel like every trans guy gets a hysto once they’ve been on T for a long time, especially because of cramping and atrophy. I honestly don’t want a hysto if I can avoid it completely, I just don’t want a major surgery like that tbh. Would atrophy and cramping just get worse and worse? Are there any other risks I might run into? It has to be similar to women going through menopause who don’t get a hysto, what happens to them?
