Breakthrough bleeding after a year and a half of T

I’m having breakthrough spotting mixed with significantly more severe abdominal pain like how I would when I had periods pre-t. Is this something I should worry about? And or see my doctor about? I haven’t missed a shot and even if I do the spotting only happens for a day but it’s been quite a few days with the accompanying severe cramps. My doctor says I have atrophy and will need a hysto in the coming year but I’m worried this could mean I need one sooner. Is this something I should contact my doctor about? Or is breakthrough bleeding relatively normal?
