TLDR: Skip to the end for my list of questions.

I was circumcised very tightly. RCI-0, super tight cut, does not even begin to describe it. When flaccid my penis had very little skin mobility. Even in its most flaccid state I still couldn’t pull my shaft skin over the glans. When erect hair and my scrotum were pulled up the shaft to compensate for loss of tissue. Erections were painful. Because the circumcision was not performed evenly my penis curved to the left and my glans tilted awkwardly downward. The scar was jagged and ugly. I think the worst complication is that I have a severe penoscrotal webbing, commonly called a turkey neck, that goes over half way up my penis.

When I began to masturbate as a child I didn’t have access to lube and could not do it the typical way by making a fist around my penis because of the lack of skin mobility. The only way I could masturbate was by pinching the skin just below my glans with my thumb and index finger of both hands and jiggling back and forth. Things got so much worse when puberty started and my penis began to grow. The tiny amount of skin mobility I had disappeared and erections became increasingly painful. I never developed "low hanging" balls. My scrotum was always tight. When I got an erection the penoscrotal webbing pulled my tight scrotum and testicles forward uncomfortably and looked awful. Penetrative sex was difficult and painful.

My earliest memories of erections are the pain from not having enough skin. Since I didn’t have any other frame of reference I thought it was normal until I discovered foreskin restoration on the internet. I have been restoring now for several years but it is a slow process especially considering my starting point. Since I didn’t have enough slack skin for devices I use manual method 2. It is still tight but at least it is no longer painful. My best guess is that I am now an RCI-2, Medium cut. When flaccid my inner foreskin bunches up just a little behind the corona but will not cover it. When erect I have just a little skin mobility that allows me to be able to pull skin to coronal ridge but not over it. I can now masturbate by gripping my penis with a fist and by pulling the slack skin as hard as possible am just able to pull my foreskin up to the coronal ridge. Oh my God, I have never experienced so much pleasure from my penis. Just touching my foreskin to my coronal ridge is so pleasurable that I can’t wait until I have an RCI-9, full erect coverage where my foreskin will stay in place even during full erections and I can experience my foreskin gliding over my entire glans. My penoscrotal webbing still pulls my tight scrotum and testicles forward uncomfortably and looks awful.

If I could have surgery to design my ideal penis with what I have and what is possible I would completely remove the penoscrotal webbing and get recircumcised to make the amount of shaft skin and foreskin even and remove the nasty looking scar. Unfortunately I don’t want to give up the progress I’ve made to get recircumcised so I might consider it after I have restored a lot more skin. I know that restoring should help reduce a lot of the problems with my penoscrotal webbing but it will take a long time, it won’t make it fully go away, it will probably still look ugly, I don’t want to have an ugly dysfunctional dick for many more years, and I can get it removed without setting back my restoration process.

So with that in mind a few months ago I saw a urologist about a scrotoplasty. The webbing itself doesn’t affect penis size but it can affect the perception of it. When he examined me he talked about it being one of the complications and that it can make people mistakenly believe that you have a small penis. It made me feel a little better when he said, "but nobody would make that mistake with you." Scrotoplasty is generally considered a cosmetic procedure and not covered by insurance unless there is a medical reason for it. So I tried explaining the problems it was causing: penetrative sex is difficult and painful, it gets caught in sex toys which is uncomfortable or painful, and it pushes condoms up my penis to the point where they slip off. He said if I was using condoms to prevent pregnancy then the routine solution was a vasectomy. I explained I was using them for protection so that was not an option.

Unfortunately he had never done a scrotoplasty and referred me to another doctor. I decided to do some more research first. I found a plastic surgeon in Atlanta that appears to be a specialist but he charges $8,500 for the procedure. He doesn’t take insurance but even if he did it would be out of network and not covered even if necessary. I really want a solution because on top of all the other problems it causes the biggest one right now is that it often causes my testicles to get sucked into my Bathmate penis pump which is slowing my penis enlargement process. I kind of want to wait and see if the problem is resolved by restoring my foreskin before resorting to surgery but that is so many years away. I really wish that I could get the instant fix that surgery offers but with all the other problems with my penis I'm terrified of having a bad outcome. I feel like my dick is already ugly and if surgery made it worse I would be devastated.

Here’s what I’m looking for:

  1. I have found little to no experiences and reviews of penoscrotal webbing surgical removal. If you have experience please share.
  2. For guys that have a penoscrotal webbing and have done foreskin restoration, what has your experience been? Does complete restoration resolve the problems or do some of them remain?
  3. I have tried 4 or 5 different devices but I don’t have enough slack skin for any of them. Part of the problem might be that I have a large flared mushroom shaped glans. Even though it seems like I have enough inner foreskin to roll over the coronal ridge I am unable to do it. I have contacted some device manufacturers who have told me they can’t help me or that they are working on products for guys with a large glans but none of them have been released. Guys who have a large glans, what has been your experience? Any advice?
