Fire Marshal dinged us on the fact that you can kind of see the edges of a previous hole there that isn't totally covered by the blank plate or smoke. It's something we inherited, so I can't tell you why there's a blank plate or why the smoke is off center, but I'm guessing they were trying to cover as much of what was left behind after removing the previous device as possible. I don't know what NFPA has to say about this and a cursory search didn't give me much.
