What were the actual targets of the bombs? And what is actually safe in the world after they fell?

From my understanding, it was mostly China and the USA at war with each other. Either one sent the first bomb and the rest soon followed. Now I know the US has even nuclear weapons to destroy the world on its own, and I assume China does as well. So what would be their targets? I doubt China would send a nuke to somewhere like Idaho and I find it hard to believe there’s any strategic value in West Virginia (apart from the conveniently created launch sites in-game). New York, Washington DC, Boston, Philadelphia, Los Angeles, Seattle, Houston, Denver, and Chicago I can see as all strategic targets with their surrounding areas being in the blast zone. And the same with China. Sure the coasts are major power centers but like anywhere inland towards Tibet or Mongolia seems like a waste. Plus if it’s just the two countries at war, wouldn’t places like Africa, the Middle East, South America, and parts of Europe be safe from the bombs and the fallout?
