Question: What makes a belt balancer throughput limited?

Ive been seeing lots of posts about balancer designs that are throughput unlimited, but I dont understand what makes a more simple design have limited throughput? The only way I could think that that would happen would be if theres a bottleneck in the balancer, but I dont see how thats possible, outside of N to M where N is bigger than M.
