ELI5: Why are over the counter medicine dosages the same for a 4'8" 95lb female and 6'8" 275lb male?

I'm talking like acetaminophen, ibuprofen, benadryl etc.

Oral medications are processed through the liver. Therefore the liver loading is the main consideration when dosing. This is different to IV medication that bypasses the liver and is calculate based on the size off the weight of the individual. Not to mention a medical professional will be the one considering your dosage, with over the counter the drug companies have to treat people like idiots and give extremely simple dosages that are standardised.
