ELI5: Does medical marijuana differ from "regular" marijuana, and how?

I'm just thinking, just because it's prescribed by your doctor, it shouldn't logically stop you from going into a hash psychosis? Is it because it's less potent, taken in smaller doses? And do medical marijuana users still experience the recreational effects?
