Accidental and purposeful strangulation, breaking the neck, throat slitting, etc. are extremely common and effective causes of death, and while knives and ligatures may be modern human inventions, violence is nothing new, predators have been going for their preys' necks for millions of years, and accidental falls have been a thing since the beginning of gravity (aka always). I can't speak for reptiles or even all mammals, but why haven't necks in general evolved to be sturdier and better protected?
