Got to push TXA for the first time last night on a penile bleed secondary to a pulled catheter. It worked like a charm. TXA now has the Jewtino Medic seal of approval.

79yo post surgery sent home w/ a catheter. Hx of afib w/ thinners. He felt the urge to pee due to the catheter placement and for some reason thought the catheter was obstructing his ability, so he pulled that fucker out. We get on scene and there's about 250ml on the floor and a little waterfall of blood pouring from his meatus.

AOx3, initial BP of 83/58 w/ Afib on the monitor. 2gm TXA, 8mg zofran for nausea, 1L bolus, and then another 700ml over the next hour during tx. Final BP was 106/70, bleeding slowed to a trickle then stopped completely by the second recheck 20 min into tx.

Overall, I'd give the call a 6/10 for being able to give a new to me drug and shock management, but definite points lost for genitals.
