The Ebonheart Pact consists of Morrowind, Black Marsh and Skyrim. However, House Telvanni of Morrowind refused to sign the Ebonheart Pact, only Shadowfen and Thornmarsh of Blackmarsh ended up joining the Pact, and Haafingaar Hold, Hjaalmarch Hold, the Reach, Whiterun Hold and Falkreath Hold split off from Skyrim to form Western Skyrim base in Solitude with High King Svagrim. The Reach ended up getting overrun by Reachmen who set up their own governent based in Markarth, and are trying to overrun Falkreath Hold. Black Marsh consists of Saxhleel (Argonian) tribes, Murkmire was assumed to be part of the Pact, however it proved to be a wrong assumption. The Ashlands, Molag Amur and Sheogorad Isles of Vvardenfell in Morrowind consists of nomadic Ashlander tribes

The Aldmeri Dominion consists of Summerset Isles, Valenwood, Anequina and Pelitine. However, Arenthia of Reaper's March in Valenwood ended up getting occupied by the city-state Skingrad which retained it's legion after the Planemeld, and Euraxia Tharn murdered the royal family in Rimmen in Anequina and ended up usurping the city. Pelitine is in anarchy after the Knahaten Flu which killed off the royal family in Senchal, however Imperials who remained after the Planemeld govern the city atleast, with pressumably aid from the Aldmeri Dominion. We don't know anything about Tenmar Forest and ZOS made the area between Northern Elsway and Southern Elsweyr too thin to add a new zone there, so we can assume it is part of the Dominion.

The Daggerfall Covenant consists of High Rock, the city-state Orsinium and some Forebears territories in Hammerfell. However, Bankorai contains Imperial remnants which lay claim to the region. We don't know much about Hamerfell outside of Alik'r, Bankorai and Hew's Bane, however we can assume the areas are dotted with city states and there is no centralized Crowns or Forebears kingdom outside of king Fahara'jad of Sentinel who rules over Alik'r and Bankorai as part of the Daggerfall Covenant. Abah's Landing does not seem like the capital of a kindom which rules over Hew's Bane or beyond, it seems more like a city-state.

The green dots are Imperial cities still standing after the Planemeld, either governed by local city-state autorities like a count, councilor or legion, or foreign powers like the Pact, Dominion or covenant. The Gold Coast split off from the Empire after the Planemeld, however it is not its own kingdom, and Anvil and Kvatch seem more like rivaling city-states. Same thing with Blackwood, Leyawiin and Gideon seem more like city-states than seats of centralized power, but atleast Leyawiin and Gideon cooperate together.
