So something that I’ve always been intrigued by is that during 12’s final monologue before he regenerates, he mentions the fact that only children can hear his name, when the stars are right and the night is quiet (or something to that effect). This has had my head going ever since I watched Twice Upon A Time. After a good long while of thinking, I think I have something put together that not only makes sense in universe, but also in the real world, and it effectively makes The Doctor a true hero in my eyes.
So my interpretation of 12’s lines are somewhat literal, and somewhat metaphorical. I do truly believe that children, no matter what time or place in space, can hear The Doctor’s real name. But, they don’t hear it exactly how it would be pronounced in Gallifreian, or even how it would exactly be pronounced if it was translated to Earth English. Instead, I think that this actually means that, if we were to translate The Doctor’s real name into the simplest translation of 21st century English, it would translate to Hope. This to me makes sense because we learned during 11’s run that The Doctor could never turn and walk away at the sight of a crying child. This shows just how much sympathy The Doctor has for not just people, but specifically children. And I think it’s because children are truly the only people capable of having genuine hope. Children can be put into the worst situations, but they will still smile and hope that the coming days will be better. They still have so much life ahead of them, and The Doctor wants nothing more than to make sure that children live to see those coming days. I think this also gives another explanation behind them choosing the name The Doctor. If there’s something that most people can agree on, it’s that doctors really do their best to make children feel safe and comfortable during hard times.
Maybe this theory is just me looking for a bit more optimism in the character than there really is, but I think it's something worth thinking about.
