Here's what i got, and this is for something in the long future, so i got plenty of time to adjust or change things. Some things listed are from other books so i will quote them.

Story Bits. This poor unfortunate soul is Khargol Black eye, a titan of a man who seeks his mortal death. Long story short, he wanted to be know as the mortal god of war, and in his groups campaign they fought against an avatar and champion of The One-Eyed God (Gruumsh) and Khargol got the killing strike. For this The One-Eyed God proclaimed Khargol as his new avatar of war, stripping away his mortality and bestowing the "gift" of immortality. Now with his only goal of being the mortal embodiment of war forever gone, he roamed the material plane for some untold years before vanishing completely from his realm. In truth, Khargol was picked up by a mysterious magi traveler from a different existences using a Well of Many Worlds, the truthful promise of "A mortal death" was all Khargol needed to join his side.
