Homebrew Level 1 through 20 campaign

Hey guys! So I've had this idea for a campaign setting floating around in my head for a while, and I've decided I want this to be a start to finish campaign setting. Anyone have experience with this? What's the best way to start laying it out?
Just start at level 1 and move forward? Block stuff out into chunks of 4 levels at a time?
