Advice for someone terrified of the process of getting disability

Hi.

I(27F) have been in and out of hospitals since I was 13. I definitely qualify for disability without disclosing what I have here. My parents never put me on disability. Now that I am an adult I want to work but physically can't due to it causing my health to rapidly decline and ending up in the hospital needing surgeries (this has happened three times in the past with all my previous jobs). I have extreme social anxiety and I'm terrified to call to get disability and start the process. Every time I look up on google how to call and ask to start the process all I get in results are if I qualify for disability with anxiety which is not the advice I'm looking for.

I don't know what to do. I figured I would post this here for any tips on how I should go about applying, if I should ask for any accommodations for the social anxiety, if it would be okay if someone else called for me to make that first step or if I need to buckle down and do it myself.

Also I apologize if this kind of post doesn't belong here. Thank you for anyone that reads this
