This world is much like ours, but with some important differences.
For one, girls and women all wear diapers, and instead of women's restrooms, there are diaper changing rooms, with one or more male attendants who take care of the business of cleaning up the girls. This is something that just isn't really questioned. Men use toilets, women use diapers. It's just how things are.
Secondly, trans people are more accepted here than they are in our reality. But that doesn't mean they're fully accepted. Sure, a trans girl won't be called a man, nobody bats an eye at her wearing skirts, dresses, make-up, or, well, diapers...but they're still seen as inferior to "real" women. Trans girls are often called "dickchicks", bullied, and are expected to submit to superior women. Even people in positions of authority, like teachers, doctors, etc, often share this sentiment.
I would play a trans girl in this world, and you...well, I have several ideas, maybe we could just play a sort of "collage" of different scens in my life. Some ideas would be being bullied by girls in the school diaper changing room, my (step-)mom showing clear favoritism towards my cis girl (step-)sister, maybe a scene where I'm being humiliated by a medical professional...
