First, the scene could only focus on nurse's meeting and not much after. Medical fetishes, chastity, edging, testing, teasing, pegging, caging, all that work. I'd imagine she hasn't worked with slightly older man or a foreigner, so there is plenty of chances for dialogue, arguing, humiliation and little extra work that really wasn't that necessary.

Second, fans of long term play could take instead of role of some sort of guardian, guide, handler or so who oversees this man now out of his depth. It's more of a traditional D/s scene with focus on long term training and control. More worldbuilding could support this theme.
