Frozen shoulder - does it go away?

My right shoulder/upper arm has been killing me for ages.
Us T1s are at greater risk and I've read that a frozen shoulder:
- Starts
- Gets worse
- Reaches a peak
- Gets a bit better
- Goes away
Over a period of two to three years. I'm on about year four and it's still there :-(
I do the stretches and the exercises and the pain relief but I'm curious to hear if anyone has had surgery for this? Golf has long since stopped and it's now affecting my ability to play snooker as I can't full stretch my resting arm :-(
