my derealization is so bad and i dont know what to do

im extremely derealized all the time. even memories dont feel real and it feels like everything is a dream. it feels like im genuinely in a different world and im having severe paranoia to the point im afraid its going to become delusion. this is unbearable and now im feeling a weird pressure in my throat and im getting really anxious about it. please help i dont know what to do
