How do I bring up that I'm having surgery in a few months to my Dad?

Obviously I'm not going to tell him until right before, but with the dementia and his late stage I don't know how to bring it up. He is currently living with my brother and sister in law and I visit often, but every time I do he tells me he was wondering where I was and he is worried (he's always been the doting Dad and I'm his only girl.)

I'm hopefully having weight loss surgery in January if everything works out. I would be hospitalized for 1 to 2 days and then wouldn't be able to see him for a few weeks while I healed as their house has a lot of stairs. I could video chat with him but I don't know if he'll understand it at that point as he is progressing rapidly and is on hospice.
