Survivors Getting Downed and Screaming w/o Deep Wound?

Hi, I've been playing Cenobite for the past week and a half, and twice now, on two different maps, I have had a survivor get downed while I'm not around them and scream to reveal their location. Different add-ons both times, neither of which applied Deep Wound; once with the Fang, but even that will only injure a healthy survivor. Even if they're on a toxic hook, that's just going to apply Mangled/Hemorrhage.
What gives? Why are survivors being downed without me?
