Are hookups real? How to?

I'm curious - I've had maybe one hookup in my life. I'm 29, male, attractive, and decently well spoken. But I have no idea how to indicate to someone that I'd like to have a hookup. I've slept with 30 people in my life but always with partners and friends or after long premeditated dates.
Do people actually meet at bars or parties and end up hooking up that night? I would love to learn the secrets. Or is this mostly a myth?
