Desperately need advice!

Me 20M and Gf 19F have a really great relationship in every aspect including sex but I constantly fantasize about her with another guy. When we have sex, when I watch porn, even when doing everyday regular activities I find myself thinking about her with another guy. How do I tell her? I have almost brought it up in conversation when we are talking about our fetishes but just didn’t have the confidence to do so. I was worried she may think negatively of me. Maybe a better idea would be to suggest a MFM threesome at first? Also are we too young? Should I wait some years and then bring this up?
