For a traditional lighter, using a glass rose stem, with an average size flame, I'd say you should have the end of the flame about 1/16" away from the tip of the pipe, and just below it. The flame not touching it at all, bc the hottest part is just above the visible flame. I prefer my chore to be no more than 1/8" away from the end of the stem, which for reference is the same size as the metal input end of a phone charger. Don't tip your pipe to the ground at all, period. Start with it vertical, melt the work until it stops crackling, and then hold it parallel with the ground. I recommend holding it back up and down while it cools.
