Need help finding cookware that meets my requirements

I'm looking for a casserole dish/baking dish/pot kind of thing that meets the following requirements:
- Is both stove top and oven safe
- Is about 40cm/16in long (so that I can have it over 2 burners at the same time)
- Comes with a lid
- Is at least a few inches deep (so that I can braise/make stew) Ideally the deeper the better, to reduce splashing when browning
Prefer if it was cast iron or something with good heat characteristics.
