TLDR: Have Chronic Epiditimytis or so I think. Want to have a vasectomy but concerned of lingering inflammation. NYU doc sent me for second opinion to more seasoned NYU surgeon. 2nd opinion resulted in appointment for procedure but without examination, no review of records, minimization of my concerns and a procedural plan that involved open ended vasectomy with a steroid injection. Doc was a bit wired, talked over me into a voice recorder and fidgeted too much. Should I consider a third opinion?
Meat and potatoes: So about 6 years ago my left testicle swole up to the size of a plum. It was crazy painful. Imaging and follow up with the Uro NP showed that an infection caused epididymitis.
Treatment took care of the infection and showed negative in urine. However several months after I still had pain and tenderness and it was making me lose sleep.
NP put me on two other courses of antibiotics and 30 day regiment of antinflammatories.
Pain and tenderness always came back. Follow up imaging showed inflammation of the epyditimus and somethong like a hydrocele or whatever that they weren't to worried about. He told me chronic epididymitis was a thing and that the area is very sensitive. The pandemic hit so I never followed through on testing sperm.
Fast forward and I want a vasectomy. Epiditimytis still present but there's mostly discomfort unless there's a flare up. Don't know the triggers to the flare ups. Consulted an NYU doctor out of Nassau LI and he told me that my case is they cautionary tail of vasectomy patients that could have painful outcomes. Said I should seriously consider second opinion from his colleague with 15 years of experience. Brief manual examination was conducted. He couldn't really reproduce the pain but I told him I could.
2nd doctor consult was virtual. Lasted all of 15 minutes. He asked me how he could help me and I told him my story. While I was talking he would interject without warning and talk his notes into a microphone. The entire time his eye contact was inconsistent, he was fidgety, and kind of dismissive of my questions and concerns. He seemed odd, like he was up on something but it could very well just had been coffee hitters.
So I know surgeons, even some of the good ones have a reputation of having bad bedside manner, but I fear he may be overlooking something and just wants to snip. He said he would take a look while inside and inject a steroid for the inflammation.
He only does open ended. Larger opening on left and very small on the right which feels pretty normal in comparison. When I tried asking about other procedures he told me he only does the one, he's done it 1000's of times and not to worry that I'm in good hands.
Every part of me wants to get this over with but I have the doubt.
What do y'all think?
