I need some career advice, I am 36 and I need to switch careers, can someone help me?

I was an executive chef most of my adult life. But like most people in my trade I became burnt out, developed addictions, lost my wife and kids... I’m 36 now, I did 3 months of inpatient rehab and I’ve done about a year and a half of outpatient rehab. I want to rejoin the workforce before anymore of my good years are gone. Like I said the only thing I have EVER done is cook. I have no degrees or certificates outside of my industry. I left my trade at 62k. I know there’s no way I’m going to make any where near that for quite awhile. I’m not very good at math or computers but I like to think I’m above average intelligence. What should I do? Oh, and I have multiple physical impairments from the years of drugs and alcohol, so I would prefer work from home jobs, or at least work from home as much as possible.
