Olá, boa noite!

Eu to tentando recriar essa interface, mas o meu problema ta com a parte dos 3 itens do meio, que são itens de opção e calendario, to tentando utilizar um esqueleto que encontrei no desing material, porém eu não to conseguindo implementar no código (nem no xml e no código da classe), e tentei pelo spinner, mas tbm não to conseguindo finalizar, alguém consegue me ajudar com esses 3 itens por favor?
(É em kotlin)
