Why did my sculpt turn out so granular?

The first picture is where I started from. I then used the subdivision modifier to create the mesh in picture 3 and used the smoothing tool to smooth out the angular surfaces. The endresult is picture 2.

I then turned it into an stl for printing and when I loaded it into my slicer (lychee) it turned very granular and weird, looking nothing like what I had in blender.

Auto-smooth is turned off. How can I fix this? Do I need to subdivide the mesh even more and try the smoothing tool again?
