Ultra Hard challenge tips

So I'm attempting to do the Ultra Hard challenge (it's one of the last few challenges I need to do and I just want to cross it off so I don't have to worry about it until my next two save files) on the first floor I got Judas's Shadow and I activated as soon as I could. I got a deal with the devil and there is an item for 2 hearts. Should I take deal with the devil items or go for deal with the angels. As of right now my health is 1 heart, 2 black hearts and 3 soul hearts.
