How can I leave my high needs baby?

So, my kid is what I would consider high needs. She pretty much requires being held 24/7, although she'll tolerate being in her swing for short periods of time, and does okay on the floor for a few minutes, if someone is playing with her. She won't sleep in her crib or Arm's Reach yet for longer than 5 minutes at a time, so since birth we've slept together in a recliner at night. That sounds crazy, but if we do that, she sleeps in 5-6 hour chunks at night, and I've been willing to sacrifice the bed for the sleep. It's very hard to soothe her without popping a boob in her mouth, and thankfully I've had a nice long maternity leave to be able to do this for her.

The problem is that my leave will be over in two weeks, when she's 12 weeks old. I need advice on: 1) Transitioning her to being able to sleep in her cosleeper; and 2) Being able to be soothed by something other than the boob.

Relevant details: she's currently bottle fed by alternating formula and expressed milk, and then I let her free feed from the breast on demand between bottles (4oz every 4 hours or so). Her grandpa will be the one watching her until the first of the year, which is when she'll start daycare. I'm not worried about daycare, since they know babies, but my stepdad hasn't cared for a baby in 21 years, so I need some things he can do for her, and if I need to get her used to other forms of soothing, I've got the next 2 weeks to do it.
