Fin rot with good parameters?

My betta Castor has had fin rot for about a momth now and the severity has gotten to the point where even my boyfriend noticed it this morning. I do weekly water changes of 30%, test frequently (most recent results were 0ppm Ammonia, 0ppm NO3 0ppm NO2, always ask boyfriend to verify), and I'm struggling to see what I'm doing wrong. Is there anything else I can do or should I set up a hospital tank and dose with antibiotics?
