Why does it sometimes work and sometimes not?

I take 4mg Valium/diazepam here and there when I need it. Usually it works good , not great , I feel like I’d benefit from a higher dosage. The only time I’ve felt a full effect is the very first time I took it, I’ve never felt better , best feeling in the world. Now it’s meh, just makes life easier. Occasionally though I’ve still had a panic attack whilst taking my 4mg. How come sometimes it works and sometimes not? Surely I’m not developing a tolerance? I even remember my third time taking it, it felt no where near as strong as the first. I don’t want to have to increase because I feel like that’s a slippery slope! Can anyone relate?
