How do I politely decline the extra things they say my car needs when I get an oil change?

I found a place I like to take my car for an oil change. They do a lengthy inspection of the car while doing the oil change. Then, they present the issues they found and offer to do the work needed. How seriously do you take the suggestions like these when you get an oil change? I had an inspection like this done when I went to the dealership once for a repair. When I later mentioned one of the items to my local mechanic, he said that it didn't need to be done at all. How do you guys handle this?
