When do you figure the medicine isn’t working?

I’m 36 female, was diagnosed adhd roughly 6 months ago and have been on dexamphetimine for roughly 5 months now. I started on 5mg per day and have slowly been increasing with doctors supervision and am currently on 20mg per day (2 in the morning and 2 at lunch time). At what stage would you cut your loses and accept that this medication isn’t working for you? I’m noticing little to no difference in what I believe are my adhd “isms” and I don’t know if it’s one of those things where a. I need to increase again b. This medicine isn’t working for me and I need to change to an alternative or c. Maybe the “isms” aren’t something that can be “helped” with medicine/maybe I was misdiagnosed. Not sure if it makes any difference, but I am also medicated for depression and anxiety with Effexor 150mg and have been for 15 years
