[USA] Obamacare: What happens to employees under Religious Exemptions to Contraceptive Coverage?

If I'm reading the existing religious exemption correctly, employees still get free contraceptives when their employer has a religious exemption.

In practice, what is the difference between employees (and employers) under the exemption and those not? Are employees guaranteed the same level of care? Are exempt groups paying less for the same level of care?

The cost offsetting is done "by adjusting the user fees paid in connection with a federally facilitated Exchange"--does this mean the prices of all plans go up to compensate for exempt groups? Do prices only go up for non-exempt groups?
