Interesting patient

Doctors, I once again have another interesting case I hope you can help me with. Pt is a 15 y/o F with respiratory distress. Pt is also showing signs of bloating and cyanosis which when combined with the chief complaint would make a case for Chronic Bronchitis. This would be the first case of juvenile chronic bronchitis I have ever heard of. I can't think of any other reason's why this young woman would look like this. Help me out.
