Do you need a separate Dr for your physician form to apply or can the Cannadoc do it?

I was hoping to get my medical card but I have no physician written certification. I was curious if I'd need to in person a different doctor or in person the one I apply to for the medical card or if tele was fine for the appointment.

I'd had it in CO for my kneecap coming off my leg but that isn't a thing here but I do have PTSD with 2yrs of therapy in college noticing my flashbacks, lack of emotion and struggle to be present at times. I doubt this is good enough and it's been 10yrs since I stopped therapy.
