[Steam] Don't always get achievements when I should- why?

So I've been playing some offline games recently (not looking for lecture about why playing online is better, I'm happy playing offline at moderate/hard or online with/against my brother and a friend) and trying to get some of the achievements- namely the 'play every map', and 'defeat/win as ___ civ up to 10 times' ones.

However, a couple of times when defeating a certain civ for the first time I wouldn't get the award for it. I have been having some internet issues in recent weeks- do I HAVE to be online for the game to score it? Are there any other known issues? Both times I didn't get them I was playing against a variety of different civs to get more done at once but I'm sure I've done that before and they've counted before.

Also, I was wondering where I could go for help/advice on the campaign missions- HD & Forgotten. I've beaten the first of each and a few of the 2nd/3rd missions but I'm struggling with many.

Edit: Just finished a game I started last night while offline, and was online for the last half hour this morning. But looking back at achievements it still says I have 0 wins using the Byzantines. Going to try the exact same game (rules/settings etc) again online the whole time.
