Refunds on Business Class Lowest

I’ve been planning a trip to Osaka KIX from YVR and I’m in a bit of a conundrum. I looked at Premium Economy Flexible first and that one is 100% refundable but then I saw how close the price was to Business Class Lowest but there is a fee for a refund. Is it $200 per person? I saw that somewhere in their website. Reason is that it’s my wife’s desire to go on a trip and enjoy life while having hemodialysis done there and we want to play it safe if something comes up.

Now booking Business Class Lowest would save me $600 over Premium Economy Flexible but would it be worth it?
