AD issues with accounts locking out. Possibly due to Admin Count parameter?

If anyone can help it would extremely appreciated. Our issues:

  1. Accounts being locked out over and over and over again. Not sure what is causing it but looks like Exchange/outlook might be involved. Outlook is quit but account still keeps locking out.
  2. Accounts showing as Admin even though they should just be normal user accounts. The Admin count attribute is set to 1 when some of them are not Admins. How can we stop this from happening?

They both might be related but I am not sure. I know that Exchange and AD have issues working together when the account is a Domain Admin but I'm not sure if Admin count = Domain Admin.

UPDATE: Just a little more information. None of these users are supposed to have Admin of any type nor be in any admin type group. They also had this happen to their AD account. The only consistency is that they all have had the Security -> Advanced -> Inheritance turned off. I turned it back on and it didn't help at all though. But it was the same for everyone having this issue.
