Travel Nurse Injured On The Job

I'm at a bit of a loss, and don't know how to proceed. Any help would be greatly appreciated.

I'm a travel nurse. My permanent residence is in Florida. I'm working for a staffing company based in Colorado. I was injured by a patient at the hospital I worked at in Vermont: I was attacked, and injured my neck and shoulder terribly. I've remained on-site in Vermont, and had 4 weeks of weekly urgent care visits, and have received one weekly check for about 30% of my weekly pay, and nothing for the past three weeks. The hospital cancelled my contract, for the reason that I "refuse to tell them when I will be back," but I am not cleared to work. My travel company has referred me to their insurer, and stated that all communications in this regard now need to go to them; they aren't answering my calls and emails now. The insurer won't call or email me back. They are in New York.

I'm hurt, and I can't work, insofar as my work is very physical and it hurts to lift a coffee cup. I have a referral to an orthopedist, but they haven't scheduled anything yet. I don't have a claim number, a contact person, or...really any idea how to proceed and keep from ending up homeless with a fucked shoulder.

What state workers comp do I call? Do I? Who? What? Help!
