Where and when am I supposed to find higher-tier amulets?

I am on the second to last journey, and have yet to find some of the amulets mentioned by the wiki, like the spreading rot one, or the ghost one.
I have a couple of purple ones I found as loot, and one legendary (related to the heathen), yet nothing else and I am nearly done with the game; did a patch change drop rates, or is it tied to something else like reputation?
